Liability Waiver

On behalf of myself and the player being registered, I recognize that baseball entails serious risks. Consequently, except as noted below, I and the player being registered relinquish all rights to a claim of any kind, including the right to a claim for bodily and material damages, regardless of the cause, against Baseball Ontario, the Provincial Womens Softball Association (PWSA), its member associations, including  Ancaster Minor Baseball and Softball Inc (AMBSA) and their respective officers, employees, coaches, umpires, assignees, agents, representatives, and sponsors, even if such damages result from negligence of Baseball Ontario, the PWSA, its member associations, including AMBSA and their respective officers, employees, coaches, umpires, assignees, agents, representatives, and sponsors.
Without restricting the generality of the preceding, I and the player being registered also relinquish the right to any claim against Baseball Ontario, the PWSA, its member associations, including AMBSA and their respective officers, employees, coaches, umpires, assignees, agents, representatives, and sponsors resulting from a decision on their part, regardless of the nature of this decision.
This release of Baseball Ontario, PWSA, AMBSA and the other persons noted above does not preclude myself or the player being registered from making a claim under any sports accident coverage provided by Baseball Ontario and/or the PWSA, and/or AMBSA to their players. An overview of the current sports accident coverage provided by PWSA can be requested.

Rowan’s Law

Under Rowan's Law, before any player can be registered with the local association and the PWSA, the player, and the parent or legal guardian of the player if the player is under 18 years of age, must review one of the Concussion Education Resources provided by the Province of Ontario and must review the Player Code of Conduct. SafeSport and Codes of conduct

Parents/Guardians/Spectators and Atheletes are subject to the Code of Conduct and Ethics found in the PWSA Safesport policies. We encourage you to review the entire policy along with the code of conduct and ethics found on page 31 through 35.

Acknowledgement:

I hereby confirm that the player being registered to participate in softball activities with the local association and PWSA, and the parent or legal guardian of the player being registered if the player is under 18 years of age, have reviewed one of the concussion education resources provided by the Province of Ontario as referenced above and have reviewed the player code of conduct as referenced above. I/we further acknowledge understanding the nature and risk of concussion and head Injuries to athletes, including risks of continuing to play after a concussion or head injury is suspected.

Terms and Conditions

In consideration of Ancaster Minor Baseball and Softball Association (AMBSA) accepting the player registration as submitted, I grant him/her permission to participate in the selected AMBSA and/or affiliate league program for which the registration has been submitted.  For the same consideration, I hereby release and forever discharge AMBSA, its Officers, Directors, Conveners, Coaches, Umpires, Affiliates Leagues or other Officials and the City of Hamilton from all claims, demands, damages, actions or causes of action arising or to arise by any reason because of my son's/daughter's participation in any AMBSA program, in this or any successive years, including (but without limiting the generality of the foregoing) any and all dental and medical bills and further of and from all claims or demands whatsoever in law or in equity which I, my heirs, executors, administrators or assignors can, shall or may have by reason aforesaid.  

The submitted registration will comply with the no refund policy posted on the Associations registration webpage.
I further agree to abide by the rules, regulations and bylaws of the AMBSA. I understand and acknowledge that AMBSA has my agreement for the collection, use and disclosure of Personal Information in accordance with Ancaster Minor Baseball and Softball Association’s Privacy Policy, in this year or any successive years of participation.

Photo/Media Consent

I understand that players may be photographed by the press or the AMBSL. These photographs or other electronic images may be released or distributed on our website, in our social media sites, in our newsletters, on promotional material & throughout media outlets, to highlight & promote the AMBSL and its accomplishments.. I understand that I may receive follow up emails from the AMBSL as a result of this registration
